OFFICIAL RULES
1. ELIGIBILITY: NO PURCHASE NECESSARY TO ENTER OR CLAIM PRIZE. A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING. The Ansell Race for the Record Scratch-Off Promotion (“Promotion”) is a trade promotion that begins on May 1, 2009 and ends on August 1, 2009. Promotion is open to individuals who are employed by authorized distributors, sellers, and retailers of Ansell Healthcare [CONFIRM DESCRIPTION] (the “Sponsor”) and who are legal residents of the 50 United States and District of Columbia who are 18 years of age or older and have reached the age of majority in their state of residence at time of entry. Employees of Sponsor, its subsidiaries, licensees, agencies, affiliates and members of the immediate families (spouses and parents, siblings, children and their spouses and in-laws) and persons living in the same household of such individuals (whether related or not) are not eligible. Entrants may be subject to rules imposed by their employers relative to their participation in trade promotions and should check with their employer for any relevant policies. Void where prohibited by law or internal business or company policy.
2. TO ENTER AND PLAY: Play the Scratch-Off Ticket obtained from [INSERT INFORMATION]. If Scratch-Off Ticket indicates you are an instant winner, you win/receive the prize indicated, subject to verification, as described below. You may also obtain a Scratch-Off Ticket from Sponsor directly by sending a self-addressed, stamped envelope (VT residents may omit return postage) to: [INSERT ADDRESS]. Requests must be postmarked by [INSERT DATE] and received by August 1, 2009. Limit one (1) request per outer envelope. No mechanically reproduced/photocopied requests accepted. The odds of winning any prize are 1:8.

All prizes will be awarded. Taxes on prizes and all expenses related to acceptance and use of prizes are the sole responsibility of winners. No prize transfer. No prize substitution except by Sponsor due to unavailability, in which case a prize of comparable or greater value will be awarded.
4. TO REDEEM: Prizes must be redeemed by mailing winning Scratch-Off ticket along with winner’s name and mailing address, phone number and email address by October 31, 2009 to: Ansell Race for the Record Scratch-Off Promotion, Attn: Prize Claims, PO Box ????, Indianapolis, IN 46268. [COMPLETE ADDRESS]. All winners and prize claims are subject to verification by Sponsor whose decisions are final. Prizes will be sent to winners at the address including with winning ticket within [INSERT NUMBER] days. If prize is not claimed, is returned as undeliverable, prize may be forfeited and awarded to an alternate winner. Winners may be required to complete an Affidavit of Eligibility and Liability Release and (where legal) Publicity Release and an IRS Form W-9, which must be returned within the time period specified by Sponsor or an alternate winner may be selected.
If, by reason of a printing or other error, more prizes are claimed than the number set forth in these Official Rules, all persons making purportedly valid claims will be included in a random drawing to award the advertised number of prizes available in the prize category in question. No more than the advertised number of prizes will be awarded.
5. SECOND CHANCE DRAWING: All claimed prizes will be awarded. Any prize not properly claimed by October 31, 2009 will be awarded in a second chance random drawing to be conducted on or about [INSERT DATE]. To enter, send a plain 3x5 card with your name, mailing address, email address, and telephone number, and a written statement “I wish to enter the second chance drawing” to Second Chance Sweepstakes, Race for the Record Promotion, [INSERT ADDRESS]. Each entry must be mailed separately, postmarked by [INSERT DATE], and received by Sponsor by [INSERT DATE]. No photocopied or mechanically reproduced entries will be accepted. Odds of winning the second chance random drawing depend upon the number of eligible entries and unawarded prizes. Any prizes not awarded through the instant win program or second chance drawing will be donated to charity.
6. GENERAL: By participating in the Promotion, entrants agree (a) to these rules and decisions of Sponsor which shall be final in all respects relating to the Promotion; (b) to release and hold harmless Sponsor, its subsidiaries, affiliates and agencies, and their respective directors, officers, employees, agents and representatives from any and all claims, demands, costs, expenses, liability, losses or damages of any kind resulting from their participation in the Promotion or their acceptance, use or misuse of a prize, including liability for personal injury or death; and (c) if a winner, to the use of name and/or photograph for advertising and publicity purposes without compensation (unless prohibited by law) and agree to execute specific consent to such use.
Sponsor, its officers, directors, affiliates, subsidiaries, agents and representatives and their respective employees are not responsible for (a) any malfunctions, interruptions, miscommunications, failures, or difficulties of any kind, including, without limitation, by any human error which may occur in the operation of the Promotion; (b) lost, late, incomplete, incorrect, damaged, misdirected, illegible, postage due requests or prize claims, and (c) any condition caused by events beyond the control of Sponsor that may cause the Promotion to be disrupted or corrupted. Persons who tamper with or abuse any aspect of the Promotion or act in violation of these Official Rules will be disqualified, at Sponsor’s sole discretion. Sponsor reserves the right in its sole discretion to cancel or suspend the Promotion or any portion thereof should causes beyond control of Sponsor corrupt the administration, security or proper play of the Promotion. CAUTION: ANY ATTEMPT BY AN ENTRANT TO DELIBERATELY UNDERMINE THE LEGITIMATE OPERATION OF THE PROMOTION MAY BE A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES FROM ANY SUCH PERSON TO THE FULLEST EXTENT PERMITTED BY LAW. In no event will Sponsor, its subsidiaries, affiliates and agencies, and their respective directors, officers, employees, agents and representatives be responsible or liable for any damages or losses of any kind, including direct, indirect, incidental, consequential or punitive damages arising out of your participation in the Promotion or use of any prize. Without limiting the foregoing, everything associated with this Promotion is provided “as is” without warranty of any kind, either expressed or implied, including but not limited to, the implied warranties of merchantability, fitness for a particular purpose or non-infringement. Information submitted with an entry or prize claim is subject to the Privacy Policy stated on the Web site. Entries become the property of Sponsor and will not be returned.
7. USE OF INFORMATION: Information submitted by entrants will be used in accordance with Sponsor’s privacy policy, available at http://www.ansellpro.com.

TO ENTER: Concurrent with this Sweepstakes, Sponsor is running a trade promotion entitled “Ansell Race for the Record Scratch-Off Promotion” (“Trade Promotion”), where members of the eligible trade may obtain a scratch-off ticket from [INSERT INFORMATION] or by mail. To enter the Sweepstakes, submit your Scratch-Off Ticket from the Trade Promotion along with a plain 3x5 card with your name, mailing address, email address, telephone number, and a written statement “I wish to enter the Sweepstakes” to Race for the Record Sweepstakes, [INSERT ADDRESS]. You may also enter the Sweepstakes by submitting just the 3x5 card identified above without also submitting a Scratch-Off Ticket. Entries must be postmarked by [INSERT DATE] and received by August 1, 2009. Limit one (1) entry per outer envelope. No mechanically reproduced/photocopied entries accepted. The odds of winning depend on the number of entries.
3. WINNER SELECTION AND GRAND PRIZE: One (1) winner will be randomly selected from among all eligible entries in a drawing to occur on or about [INSERT DATE]. The winner will receive a trip for two (2) to the 2009 Ansell Healthcare 150 at Chicagoland Speedway, including: two (2) tickets to the 2009 Ansell Healthcare 150 at Chicagoland Speedway (ARV $XXXX); two (2) round-trip coach airfare for winner and his/her guest to Chicago, Illinois from a major airport near winner’s home (determined by Sponsor in its sole discretion) (ARV $1,000; actual retail value may vary based on airfare fluctuations and distance between departure and destination); $200 toward a rental car (ARV $200); $50 toward gas (ARV ($50); two (2) nights in single hotel room (double occupancy) (ARV $250); and $400 toward meals (ARV $400) (total ARV $1,900).
All prizes will be awarded. Taxes on prizes and all expenses related to acceptance and use of prizes are the sole responsibility of winner. No prize transfer. No prize substitution except by Sponsor due to unavailability, in which case a prize of comparable or greater value will be awarded. All meals, taxes, gratuities, and other expenses not specified herein are the sole responsibility of the winner. If winner is unable to travel on the designated dates, prize may be forfeited in its entirety and an alternate winner may be selected.
3. WINNER NOTIFICATION: Winner will be notified after the drawing occurs on or about [INSERT DATE]. Winner is subject to verification of eligibility by Sponsor whose decisions are final. Winner may be required to complete an Affidavit of Eligibility and Liability Release and (where legal) Publicity Release and an IRS Form W-9, which must be returned within the time period specified by Sponsor or an alternate winner may be selected. Travel companion must sign and return a release of liability and publicity [INSERT NUMBER] days prior to travel or will not be permitted to accompany winner on trip. If travel companion is a minor, parent or legal guardian must sign and return release and accompany minor on trip. If selection of an alternate winner is necessary, such alternate winner will be selected by random drawing among all eligible entries. No more than the advertised number of prizes will be awarded.
